How to get started

  • Make a referral: Have the details ready? Complete our online referral form. We review the referral within 5 business days and will be in touch if there’s any further information we require.
  • Get in touch: Want to check suitability first or discuss current availability in a specific area? Contact our Intake team directly on (03) 9885 2533 between our business hours of 8.30am – 5pm (Melbourne time).

Urgent Referral?

We prioritise urgent referrals (eg. hospital discharge, falls risk or risk of re-injury) to be seen within 1-3 weeks.
